Spinal Cord Compression and Myelopathies

چکیده
Patients with signs and symptoms of acute myelopathy require urgent neurologic evaluation focused upon the identifi cation and management of treatable disorders. MRI of the spine is the imaging modality of choice to evaluate for a compressive lesion. When cord compression is present, surgical treatment is usually indicated. When compression is not detected, an analysis of precise lesion localization, nonneurological clinical features, MRI fi ndings, and serologic studies narrow the differential diagnosis. The key diagnostic considerations include demyelinating, vascular, infl ammatory, infectious, and paraneoplastic disorders. Empiric high-dose corticosteroid treatment is often indicated in noncompressive myelopathy; additional investigations are important to identify patients with relapsing or progressive disorders who may benefi t from preventive therapies. Patients whose symptoms continue to progress after initial immunosuppressive treatment may benefi t from plasmapheresis and occasionally from biopsy for defi nitive diagnosis.

Patterns of non-traumatic myelopathies in Yaounde (Cameroon): a hospital based study.
BACKGROUND The relative frequency of compressive and non-compressive myelopathies and their aetiologies have not been evaluated extensively in most sub-Saharan African countries. The case of Cameroon is studied. METHODS Admission registers and case records of patients in the neurology and neurosurgery departments of the study hospital were reviewed from January 1999 to December 2006. RESULT...

Myelopathy is a devastating neurologic complication of cancer. The resulting pain, paralysis, and incontinence can turn a patient with cancer from a functioning individual to one who is confined to a chair or bed. Early diagnosis and appropriate therapy can prevent or ameliorate these symptoms and improve both duration of survival and quality of life. Accurate neurologic assessment of patients ...

OBJECTIVE To identify the severity of neurological deficits, presence of signs of cervical pain, and the site of intervertebral disc space extrusion in 21 dogs diagnosed via magnetic resonance imaging (MRI) with a hydrated nucleus pulposus extrusion (HNPE) and compare those findings to dogs with other compressive cervical myelopathies. METHODS Medical records and MRI findings were reviewed in...

OBJECTIVES Post-traumatic intramedullary myelopathies and cavitations are well described lesions following spinal cord injury (SCI) in humans and have been described in histopathological evaluations in dogs. Human intramedullary myelopathies/cavitations are associated with severe initial SCI and deterioration of clinical signs. Canine intervertebral disc extrusions share similarities with SCI i...
